1. Mouthpiece - Follow instructions on package. Proper sterilization and molding will be necessary.
2. Cleats - Football specific cleats are necessary.
3. Athletic Cup with Girdle - Athletic cups are required. Cups with girdles work best for washing and for comfort.

1. OEC Player Contract - To be completed onsite.
2. ID Photo - 2x2 photo
3. 2010 Pop Warner Physical - All physicals must be completed by a licensed physician on the 2010 Pop Warner Physical Form. Past year forms will not be accepted.
4. Birth Certificate - Please provide the original for verification and a copy for the certification binder.
5. Three copies of YEAR-END Report Cards. Please make sure they are YEAR-END Report Cards.
6. Utility Bill - Required for address verification. Anyone living outside of the Costa Mesa/Newport boundaries MAY need to obtain a waiver should you reside in a city that offers a Pop Warner program. Should your city not offer a Pop Warner program, a waiver will not be necessary. Please inquire at time of registration for full detail.
